To switch providers from another pharmacy to ourselves, all you would need to do is create an account with us by using the following link to our website: https://www.simpleonlinepharmacy.co.uk/members/register.php
Once you have created an account, you will need to complete an assessment form for our clinical team to review. If you have received a weight loss prescription previously from a different supplier and want to continue on the treatment, we require you to provide evidence of your previous supply during the assessment form. This is to ensure a safe prescribing decision can be made with your health and safety at the forefront.
This evidence can be an image of the pharmacy label on the box of your previous prescription, a copy of your recent prescription, or the latest dispatch email from your previous supplier. We can accept any evidence of your most recent supply given that the following four points are clearly stated - your name, the name of the medication, the strength of the medication and the date the supply was made.
If you are unable to provide the required evidence, then we unfortunately would not be able to approve an order for a higher strength than the starting dose of the medication treatment ordered. This is because evidence of your previous supply is essential to make a safe prescribing decision to ensure there are no gaps in your treatment.
