If you are switching providers to continue your weight care journey, and you are wanting to order a medication strength above the starter dose, our prescribers will require evidence of your previous supply in order to ensure a safe prescribing decision.
We can accept any form of evidence given that the following four points of information are included:
- Your full name
- Name of the medication prescribed
- Strength / dose of the medication prescribed
- Date of previous prescription being issued
This evidence can be added during the assessment form of your first order with our service. If you have already placed your order, you can send the requested evidence as an email attachment to customerservices@simpleonlinepharmacy.co.uk.
If you are unable to provide the above evidence, then we unfortunately would be unable to approve your order above the starter dose. This information is required as it ensures our clinical team can prescribe the medication safely by confirming there have been no gaps in your treatment.
