Why do you need to know my age/ verify my identity?

In order for us to ensure that a safe prescribing decision is made for the medication or product you are ordering, we need to confirm you are aged 18 or over. To do this, we using a system called LexisNexis to verify your ID.

If your ID is not automatically verified when your order is initially placed, we require you to provide us with a form of ID for our Patient Care Team to manually verify your ID. The ID check may fail due to information being incorrect or not matching to official documents such as a passport or UK drivers licence, therefore manual verification is required by our team to process your order further.

 Accepted forms of ID:*

  • UK Driving Licence (Full or Provisional) - front only
  • UK or International Passport - identity information page
  • ID Card - front and reverse
  • UK Travel Visa

*The information you provide will only be used to verify your identity so that we can proceed with your order. Please do not omit or censor any information, and make sure that all the information is clearly visible. Expired documents cannot be used to verify your ID.
