Please include the following information:
- Full name
- Email address
- Phone number
- Date of birth
- Reason for leaving (optional)
- Most recent order number (your last SOP number, if available)

Please provide any of the information above that is missing from your request. Your details can then be passed to our Patient Care Team, who will review your account closure request.
Our Patient Care Team will contact you if they require any further information and will confirm once your request has been processed.
